An Efficient, Collaborative Process from First Call to First Shift

Whether you’re a client yourself, a case manager referring a new client, or a legal deputy overseeing a care package, we’ve designed a clear, efficient referral process:

1. Initial Consultation
Please speak with one of our experienced team members to discuss the support requirements and goals.

2. Online Referral Form
Complete a short form with key information and agreement to our terms.

3. In-Person Assessment
We conduct a thorough home visit and client assessment to identify the best-fit staff member.

4. Bespoke Proposal
Once you or your client confirms you wish to proceed with Snap Care, we will provide a clear support plan, candidate profiles, and a detailed breakdown of costs.

5. Agreement & Onboarding
Once approved, we initiate any training requirements, manage schedules, and start service delivery.

We continue to monitor and adjust the package as needed, working closely with all stakeholders and using the OPENPASS app for transparent, real-time updates.
