This involves collaborating with other healthcare professionals to create and implement and updating comprehensive care plans. The team leader will also provide hands-on support with the client to ensure they fully understand the client’s needs and the job roles.

2. Communication: Effective communication is crucial in support settings. The Care Team Leader serves as a central point of contact, facilitating clear communication among team members, clients, their families, and other healthcare providers. They ensure that everyone is informed about the client's condition, progress, and any changes in the care plan.
