About our client: Tom is 15 years old and lives with his Mum and Dad in their fully adapted home. Tom is a fun, engaging, and cheeky young man with a love of music, arts and crafts and spending time with his family. Tom has cerebral palsy and complex care and medical needs; he is a wheelchair user and is tube fed. He is reliant on others to meet all his health, care, and medical needs. He uses a communication book and switch, along with facial expressions, body language and sounds to support his communication. Tom has a dedicated team in place, in the coming months it will be expanding as Tom transitions to 2:1 support. The Care Team Leader role is newly created to ensure a smooth transition, and ongoing support is provided. This is a wonderful opportunity to lead a professional, experienced and dynamic team.
Overview of role: The position will be largely managerial, with the workload increasing as new team members join. Typical duties such as coordinating the team, organising the rota, training, appraisals and supervisions, managing medication and equipment, arranging appointments, timesheets, updating care plans, working in collaboration with other healthcare professionals and MDT’s, monitoring the quality of care provided in partnership with Tom’s parents and demonstrating best practice are all included. The Care Team Leader will have the opportunity to support Tom with all areas of his ADL’s, work closely with colleagues/the team; offering a lovely balance for a Team Leader who does not wish to step away fully from the hands-on care and support.
Who this job would suit: A proactive, organised and intuitive Care Team Leader with exceptionally high standards of practice. Tom requires the Team Leader to be his advocate continually promoting his independence, enabling him to direct his care. Someone confident in their ability to manage the transition ahead and dedicated to building / growing a dynamic team.
What’s great about this job: Tom is an absolute star; he is a joy to work with and captures the heart of everyone he meets. The role is highly recommended by Snap Care team members, many still on the team, or who remain in touch with Tom and his family, testament alone to what a fabulous young man Tom is, and the supportive, welcoming and professional working environment Tom’s parents, and his team have worked so hard to obtain. With a comprehensive induction process, ongoing training, and continued professional development offered –this is an opportunity not to be missed!
