About this client: Snap Care has had the pleasure of working with Jacob, 19, and his family for several years. He is a fun, sociable and happy young man with a wicked sense of humour and the most infectious giggle! Jacob has a long list of interests and hobbies, cooking, listening to music, visiting museums, speed boating, travelling, and going to the theatre, just to name a few! Jacob has cerebral palsy, is a wheelchair user and his nutrition and medication is administered via his PEG. Jacob uses Eye Gaze, sounds and facial expressions to support his communication; he has mastered expressing his likes and dislikes! He attends College during the week and has a dedicated team of SEN Support Workers in place and a Team Leader providing 2:1 support 24/7.

Overview of role: Jacob requires his SEN Support Worker to enable and facilitate him, supporting him with all aspects of his care plan, ADL’s, accessing college and his education, participating in leisure activities, hydrotherapy, keeping up with his busy social life and travelling abroad (up to 2 weeks at a time). His SEN Support Worker will assist with his rehabilitation and therapy programme (training provided by his therapy team). The night routine starts at 7.00pm, and he is generally in bed with his sleep system set up by 10.00pm. Two waking night carers are on shift at all times with Jacob, he experienced periods of wakefulness, restlessness and discomfort throughout the night, and will require regular repositioning and suctioning to ensure he is comfortable and requires close monitoring at all times, via a video monitor if not based in his bedroom. At times he may also require oxygen to be administered. The rota is provided 6 to 8 weeks in advance. Shifts are 3 x 12-hour shifts per week, including days and waking nights, starting at 7.00am or 7.00pm, and include both weekday and weekends,
