Jess is a bright and articulate 21 year old with a wide range of interests, including fossil hunting, horror movies and music technology. Jess is a creative, self-aware and capable young woman who is looking for an active, engaging and personable Support Worker / Mentor to provide 1:1 support to promote and facilitate her independence and also liaise, transport and support during any possible work experience placements.

Jess has mosaic Down’s syndrome, learning difficulties, traits of ASD, gender dysphoria and a B12 vitamin deficiency. Currently living in independent living, Jess attends college 3 days per week and spends time with her family at the weekends. Her current Snap support will be leaving the position after the 2 years and we are looking for a new person to come on board and continue the good progress.

In this role you will assist Jess with her travel to college, ensure she has everything she needs for her lessons and arrives on time for class. Jess will not need you to stay with her at all times during the learning day, however you should be on hand to assist as and when required. You will also help Jess to follow her diet, assist with shopping, meal prep and batch cook meals.

Jess would love to plan trips away for a few days, either during holidays or the weekends so we are looking for a Support Worker / Mentor who has some flexibility to provide this occasionally, as well as overnight support if required in the family home. As Jess’s advocate, you’ll be confident liaising with the college, her living community and her family.

Jess is at a very exciting time in her life however it is important that everyone involved with her care is kept updated and is working together to ensure Jess is receiving the best support. This is a unique role that will be rewarding for someone equipped to meet Jess’s evolving needs.
