Our client is a thriving case management company working with children and adults with acquired / traumatic brain injury, spinal injury, limb loss, and dementia and polytrauma across London and the South East. Case Management is a challenging but extremely rewarding role and is both varied and collaborative. A case manager is responsible for meeting each client’s individual’s needs from assessing rehabilitation needs, recruitment and managing care packages to supporting families through the litigation process.
As an assistant case manager, you’ll complete, under supervision and in collaboration with professionals, a detailed initial assessment of the care, rehabilitation and case management needs of current, new and potential clients including risk assessments; you will establish a clear case management plan and objectives for that client. You’ll provide ongoing case management support to the client, their family and care team, liaise with therapists, medical practitioners, solicitors, social services etc. Most importantly you will act as an advocate for your client and ensure they are receiving the right support in all aspects of their care plan.
Our client is looking for someone with experience in a senior role having worked within the disability/care sector. This is a client-facing role so you must be a people person, have an engaging personality, and be extremely resilient, tenacious and confident working independently. Excellent communication, IT and written skills are essential. Also essential is a driver’s license and own car; the position requires you to travel to clients homes and to various appointments.
You’ll be a self-employed consultant working for this company managing a case load of clients. Full time availability is required. However within reason and depending on client requirements hours can be tailored to suit
